Temporary Vehicle Change (TVC) Form for Special Parking Identification (SPI) Permit Holders a.k.a. NYC Parking Permits for People with Disabilities

If you need to use another vehicle temporarily that is not listed on your permit, please fill out the Temporary Vehicle Change (TVC) web form below. You may also call our Customer Service Center at (718) 433-3100 Monday through Friday, from 9 am to 5 pm. To request a TVC after business hours you may call NYC 311. A temporary change may be requested for up to 14 days. Please note that company, business, temporary, taxi, dealer, transporter or commercial plates are not authorized for TVCs.

You must report a TVC immediately before using your temporary vehicle – failure to do so may result in a summons that cannot be dismissed or your permit may be confiscated. Plates that are in judgment at the NYC Department of Finance may not be used for a TVC. Check to confirm your vehicle is not in judgment.

*Designee - If the permit holder is under 18 years old, please provide the name of the parent, guardian, or other individual having legal responsibility for the administration of applicant's day to day affairs. If the permit holder is 18 years old or older and was unable to sign the original application, please provide the name of the spouse, domestic partner, guardian, or other individual having legal responsibility for the administration of applicant's day to day affairs.
