As a Community-based Organization (CBO), if I am helping a client, how can I find out information about their application or case?

Please fill out the Authorization to Release Case Information Form and fax it to (212) 437-2615 with the accompanied inquiry. (If the client in question is a HASA or Medicaid client, you should also fill out this HIPAA-compliant form.)

After faxing the necessary forms, call the DSS OneNumber at 718-557-1399.

For SCRIE/DRIE: Call 311.

For Medicaid Renewal: Call the Department of Social Services' Medicaid Helpline at 888-692-6116.

I am interested in having a representative from DSS provide training on the various DSS programs and services. Who can I contact?

You can register for an upcoming session here. If you would like to request an individual training for your staff (10 or more attendees), please fill out the DSS Training Request Form, and someone will be in contact to schedule.

I am interested in having a representative from DSS attend my community event. Who can I contact?

Please fill out the Community Partner Event Attendance Request Form and someone will be in contact with you to organize DSS representation at your event.
