As always, you can speak with a Customer Service Representative about the Plan and your account(s) on the phone by calling at (212) 306-7760.

Please resubmit your documents as follows:

1) Inquiries and questions can be sent via email to the Plan.

2) Forms/documents can be sent via email to Please only include the last 4 digits of your Social Security number, along with your name and address on all forms. Please do not submit your form/document more than once.  This will only delay processing.  You will receive a confirmation email shortly after submitting your form/document.

3) Forms/documents can also be faxed to 844-299-2362.

4) All checks must be mailed to the Plan's Lockbox. Please use the address indicated on the form you are submitting.
