With more than 70,000 elevators and escalators in New York City, this equipment is a standard part of our lives. The Elevator Unit ensures the operational safety, reliable service and lawful use of vertical transportation devices throughout our City, that include: elevators, escalators, amusement rides, personnel hoists, dumbwaiters, material lifts, wheelchair lifts, conveyors, and other related devices. The Unit responds to complaints, conducts re-inspections of private and DOB hazardous violations, conducts audits and training, responds to and investigates accident/incidents, conducts acceptance testing, surveys, and special assignments. Additionally, the Elevator Unit processes new elevator applications, conducts plan examinations, issues permits for modernization, removals, and dismantles, as well as handles Construction Code inquiries and clarifications by mail and over the phone..

  • LIC2-EL: Elevator License Application
    Use this form to apply for or make changes to an Elevator Agency Technician/Restricted Elevator Agency Technician license or Elevator Agency Helper registration. LIC2-EL - Rev. 11/21
  • Elevator Worker Attestation
    Elevator Directors must complete this form to attest to the fact that an applicant for an Elevator Agency Technician/Restricted Elevator Agency Technician or Elevator Agency Helper is qualified and is under their direct and continuing supervision. Elevator Worker Attestation Form - Rev. 11/21
  • Elevator Application
    All new Elevator Application filings must be submitted through DOB NOW: Build. All ELV1 job filings with a current BIS job number will continue to be processed through the Elevator Unit.   • ELV1A: Amusement Ride Application

  • ELV3: Elevator Inspection/Test Report
    All new Elevator Inspection/Test Reports must be submitted through DOB NOW: Safety.

  • ELV15: Affidavit of Professional Certification
    Use this form to certify (a) the elevator application or building notice is complete and in compliance with all applicable provisions of law or rules; and (b) whether or not the application is being filed to legalize an existing condition. ELV15 Form - Rev. 12/14
  • ELV21: Temporary Use Permit Renewal Application

  • ELV22: Pre-Clearance Inspection/Test Request
    Use this form to request a pre-clearance (expedited test) inspection from the Elevator Division. ELV22 Form - Rev. 4/16
  • ELV29: Affirmation of Correction

  • ELV36: Elevator/Escalator Test Notification Form
    As of September 17, 2018, Elevator/Escalator Test Notifications must be submitted through DOB NOW: Safety.
  • B Form 10: Elevator Amendment
    Submit this form with the ELV1 form for previously filed paper filings only. All new requests must be submitted in DOB NOW: Build. B Form 10 Form - Rev. 6/09 (B Form 10 Instructions)
  • EBI1: Elevator Batch Intake (PVT only)
