With more than 70,000 elevators and escalators in New York City, this equipment is a standard part of our lives. The Elevator Unit ensures the operational safety, reliable service and lawful use of vertical transportation devices throughout our City, that include: elevators, escalators, amusement rides, personnel hoists, dumbwaiters, material lifts, wheelchair lifts, conveyors, and other related devices. The Unit responds to complaints, conducts re-inspections of private and DOB hazardous violations, conducts audits and training, responds to and investigates accident/incidents, conducts acceptance testing, surveys, and special assignments. Additionally, the Elevator Unit processes new elevator applications, conducts plan examinations, issues permits for modernization, removals, and dismantles, as well as handles Construction Code inquiries and clarifications by mail and over the phone..

Elevator Forms
- LIC2-EL: Elevator License Application
Use this form to apply for or make changes to an Elevator Agency Technician/Restricted Elevator Agency Technician license or Elevator Agency Helper registration. LIC2-EL - Rev. 11/21
- Elevator Worker Attestation

- Elevator Application
All new Elevator Application filings must be submitted through DOB NOW: Build. All ELV1 job filings with a current BIS job number will continue to be processed through the Elevator Unit. - ELV1A: Amusement Ride Application

- ELV22: Pre-Clearance Inspection/Test Request

- ELV29: Affirmation of Correction

- ELV36: Elevator/Escalator Test Notification Form
As of September 17, 2018, Elevator/Escalator Test Notifications must be submitted through DOB NOW: Safety.
- B Form 10: Elevator Amendment
