• Minimum time commitment of 150 hours and at least 6 months
• Regular weekly four to five-hour shift
• Must complete health assessment that includes tuberculosis (TB) screening and blood work for Measles, Mumps, Rubella, and Varicella (MMRV) titers.
• Must Complete mandatory orientation
  • Some programs require additional trainings
• Two references (Form)
• Application & Personal Interview (Form) **Photo ID is required
  • Walk-ins are welcome Monday, Wednesday, Friday 9:30am-3:30pm and Saturday & Sunday 10am-2pm to complete application in-person OR
  • Print application, complete and bring during above times for a personal interview.

After you've submitted a volunteer application, you will have up to three weeks to submit two reference forms. These forms may be written by either professional or personal contacts (e.g., employer, co-worker, professor, teacher, neighbor, friend, pastor-no family members please). Submit completed reference forms via email (Christina.Cornacchia@nychhc.org), fax 718-616-4782 or in person.
