Are You Qualified?

  • Minimum age: 19
  • Have a Valid DMV Chauffer's license- Class A, B, C, or E
  • You must have a valid Social Security Number
  • No fines or fees owed to NYS DMV or TLC
  • No more than 5 points on your DMV license within a 15 month period back from your most recent conviction date

Out-of-State Applicants / Licensees

NEW MANDATORY REQUIREMENT Out-of-State Driver License Applicants/Licensees (New, Renewals, and Replacements)

Effective Date: October 1, 2025

Starting on October 1, 2025, all out-of-state Applicants/Licensees—whether applying for a new, renewal, or replacement TLC Driver's License—must complete a new step to allow the TLC to obtain the necessary Client ID number for their application.

The Applicant/Licensee must follow these steps to meet this new requirement:

Step 1: Get the Required Form 

  • The Applicant/Licensee must download the ID-5 form, which is available here ID-5 Form, and bring it with them to the DMV office. The applicant/Licensee does not fill out the form.

Step 2: Visit a New York State DMV Office 

  • Take the ID-5 form to any New York State DMV office. No appointment is necessary.
  • The Applicant/Licensee must present their original identity documents (proofs of name, date of birth, and signature) to the staff for verification.
  • The DMV staff will complete the ID-5 form and take the Applicant's photo.
  • There is no charge for having your picture taken at the DMV.
  • This action allows the DMV to generate or validate the Client ID number that TLC requires.

Step 3: Get the Receipt (FS-6T) 

  • After the DMV process is complete, the Applicant/Licensee will be given a receipt called an FS-6T.
  • It is essential to keep this receipt.

Step 4: Upload to TLCUP 

  • The Applicant/Licensee must log into their TLCUP account and upload the FS-6T receipt.
  • This receipt confirms that the step to obtaining the Client ID number has been completed, and it is required to proceed with your TLC Driver’s License application.

Reminder: Applications submitted on or after October 1, 2025, will not be processed until the FS-6T receipt has been successfully uploaded to TLCUP.

Documents Needed

Before starting your application make sure you have the following documents available:

For members of NYPD and active or veteran members of the armed forces only:

  • If you work for the NYPD, as an officer: A copy of the Letter of Approval from your Commanding Officer and an Off Duty Employment Form. The form must be signed and approved by your commanding officer and the Employee Management Division.
  • If you are in the Active Military or Reserves: A copy of a certificate of current active military status and your military ID. Request records from the Department of Defense.
  • If you are a Veteran of the US Armed Forces: A copy of discharge papers or a certificate of release (Form DD214) and your Veteran ID. Request records from the US National Archives.

How to Apply

  • Start a TLC application online by visiting LARS.
  • Take a Drug Test
    • Call LabCorp (800) 923-2624 to schedule an appointment. Results take approximately 3 days.
    • Or be seen on a walk in basis at one of the locations listed on the New Driver Applicant Drug Test Information sheet.
  • Have your fingerprints and photo taken
    • Visit IdentoGO to schedule an appointment. Use service code 15425Y.
  • Complete NYSDMV 6 hour Defensive Driving Course - You must complete a New York State DMV-certified Defensive Driving Course within 90 days of submitting your online application, unless you have previously completed one no more than three (3) years prior to the date you filed your application.     • You can upload the certificate online on TLC UP.
  • Must Complete all Education Requirements within 90 days from the date of your application.
  • Pay all parking tickets - Visit Pay a Parking Ticket
  • Pay all traffic tickets - Visit New York Department of Motor Vehicles
  • Watch Sex Trafficking Awareness Training (10 minutes)

NOTE: You must complete all these steps within 90 days from the date of your application, or your application will be denied.
