How to Report

Promptly report diseases, conditions, and events as required by law. Cases of certain rare or highly infectious diseases and conditions, suspected outbreaks and emerging public health threats, must be reported to the Health Department immediately by calling the Provider Access Line (PAL) at 866-692-3641. Visit Reporting Disease and Conditions to learn which cases must be reported immediately and which must be reported within 24 hours.

Sign Up for an NYC.ID Account

NYC.ID is New York City’s Centralized Public Identity Management Application. It is the point of entry for providers to access applications, including reporting portals such as the Provider Reporting Interface and Secure Messenger (PRISM). Register for an NYC.ID account.

Review your NYC.ID profile to help keep your account secure by updating your password, managing two-factor authentication, and correcting your name if needed.

NYCMED to Be Decommissioned

NYCMED will be decommissioned this fall. Most user accounts have already been transferred to NYC.ID. Check your user profile for accuracy. If your program has not migrated to NYC.ID yet, sign up for NYCMED.

Reporting benefits you and your community. It supports the Health Department in our efforts to:

  • Facilitate any needed laboratory testing
  • Detect, investigate, and control outbreaks
  • Prevent the spread of disease through screening and prophylaxis of patients’ close contacts, including family members, and, where appropriate, sex partners
  • Prevent injury due to hazards
  • Provide feedback to providers and facilities to improve quality of care
  • Plan and monitor disease prevention and control programs
  • Quantify disease burden and monitor trends that may impact clinical care
