Replacing Reporting Central

Reporting Central will be replaced by the Provider Reporting Interface and Secure Messenger (PRISM) in July to August, after which PRISM will need to be used for electronic provider disease reports. A more specific date for the transition will be provided once available. Until then, continue to use Reporting Central using your NYCMED ID.

Note that an NYC.ID account will be required to access PRISM.

To make the transition as smooth as possible, we recommend registering for an NYC.ID account now. Register for an NYC.ID online. Registration only takes a few minutes.


Cases of certain rare or highly infectious diseases and conditions, in addition to suspected outbreaks and other potential public health threats, must be reported to the Health Department immediately by calling the Provider Access Line (PAL) at 866-692-3641.

Other cases can be reported to the Health Department within 24 hours by logging in to Reporting Central via NYCMED (If necessary, these cases can also be reported by mailing or faxing the paper Universal Reporting Form (PDF).

If you do not already have a NYCMED account you will need to register. Once you are logged in, Reporting Central can be found in the ‘My Applications’ section.
