Provider Reporting Interface and Secure Messenger (PRISM)

New Reporting System: PRISM

PRISM replaced Reporting Central in May 2025. Access PRISM online.


Providers should use PRISM to report diseases and conditions.

Cases of certain rare or highly infectious diseases and conditions, suspected outbreaks and emerging public health threats, must be reported to the Health Department immediately by calling the Provider Access Line (PAL) at 866-692-3641. Getting Started

You must create an NYC.ID account to use PRISM. You cannot use NYCMED.

There are two ways to access PRISM using an NYC.ID:

  • If you are an NYC government employee with an email address ending in @nychhc.org or @nyc.gov, an NYC.ID account has automatically been created for you. Click the “NYC Employees” button and sign in using your full work email address and your employee password to access your existing NYC.ID account.
  • If you are not an NYC government employee with an email address ending in @nychhc.org or @nyc.gov, you must create an NYC.ID account using a verifiable email address.

Review your NYC.ID profile to help keep your account secure by updating your password, managing two-factor authentication, and correcting your name if needed.

When you first log in, complete the “My Profile” page by entering your affiliated facilities, laboratories, and providers. More than one entry is allowed if you report on behalf of multiple facilities, laboratories, or providers. Completing your profile is required and will help pre-populate your case reports.


Reporting Through PRISM

Use PRISM to report diseases, events, and conditions that do not require immediate reporting.

After you submit a report, you can download a PDF for your records using the “Download PDF” button on the PRISM home page. You will need to provide the record ID (URF ID), patient’s last name, and patient’s date of birth.

To edit a report after you submit it, use the “Make a Correction” button on the PRISM home page. You will need to provide the record ID (URF ID), patient’s last name, and patient’s date of birth.

Only the user who submitted a report can download it as a PDF or make a correction to it.

Note that your session on PRISM will time out after 60 minutes of inactivity. The application will save any entered data before timing out. However, extended periods of inactivity may result in the loss of unsaved data. Avoid opening multiple tabs, which may cause your session to time out.

Frequently Asked Questions

Why Can’t I Report a Condition or Why Do I See “Incomplete Profile”?

The first time you log in to PRISM, you must complete the “My Profile” page by verifying and completing the Reporter section and entering at least one affiliated facility, laboratory, and provider. Once the profile is completed, you can submit records by clicking “Report a Condition” on the PRISM homepage. You cannot report a condition until “My Profile” is completed.

You can use the "Edit NYC.ID Profile" button to update your name and email address. Note that emails for NYC employees ending in @nychhc.org or @nyc.gov cannot be changed.

How Can I Report on Behalf of More than One Facility, Laboratory, or Provider?

You can manage and report for multiple entities through these steps:

  1. Set up in “My Profile”: All facilities, laboratories, or providers must be added to your My Profile section first.
  2. Select during reporting: Once you save all needed entities to your profile, they will appear in the dropdown menus when you complete a report.

To ensure information is saved for future use, always make edits or add new entries directly in the “My Profile” section. Changes made within an active report submission will not be saved to your permanent profile.

You may save a maximum of 99 entries per list. Do not delete all entries from your profile. At least one facility or provider must remain at all times to avoid the “Incomplete Profile” error.

How Do I Know if a Report Has Been Successfully Submitted?

Your report is only submitted once you see the confirmation page. To reach this page, you must click “Submit” twice. The report is not submitted until this final screen appears with the text “Thank you for submitting this report” and a record ID number.

Save your unique record ID (for example, 5003987). This number is required for any future inquiries or corrections regarding the submission.

Once you reach the confirmation page, you can choose from the following features:

  • View/Download a PDF version of this report: create a PDF that you can save for your records.
  • Modify/correct the submitted report: edit the report and update any information.
  • Report another condition for the same patient: create a new report for a different disease for the same person. The system will pre-fill the patient’s data to save you time.
  • Report a condition for a different patient: start a new blank report for a new person.

Immediate Consultation and Reporting

For immediate consultation and to report suspected or confirmed cases of diseases that require immediate notification, call the Provider Access Line at 866-692-3641 or 347-396-7995.

Note that you may be required to complete an online report after calling the Provider Access Line.
