Finding the right help to meet long-term care needs can be frustrating, but NY Connects makes it easier for people of all ages and income levels to find help and lead independent lives. The New York City Department for the Aging works with community partners to run the free statewide program in the City.
NY Connects is for older adults; younger adults with a long-term disability; caregivers of older adults and parents of children with a disability; and professional service providers working with someone in need of long-term services and supports.
Trained specialists can connect you with services and provide information, referrals, and support.
