A Virtual Tour of the Gouverneur Nursing Facility
Our interdisciplinary teams of healthcare professionals develop customized care plans for each resident. We are committed to providing residents with the opportunity to continue their lives with dignity and meaning, and provide individualized attention and specialized treatment. Families are encouraged to participate in developing the care plans, and attend planning meetings where they meet with healthcare professionals from each department who are directly involved with the care of their loved one. In support of this principle, the organization serves the needs of the residents by providing the widest spectrum of healthcare and services at the level their needs demand. Residents benefit from the proximity of our Ambulatory Care Center, because it enables them to receive many on-site services including internal medicine, ophthalmology, gynecology, podiatry, imaging and dentistry.
Gouverneur residents enjoy:
Chinese, English and Spanish menus, with an emphasis on individual food preferences
Regularly scheduled Buddhist, Protestant, Pentecostal, Catholic and Jewish services available, as well as cultural heritage celebrations
Telephones at each bedside
Full-time barber and beautician
Our Many Levels of Care
Short-term Rehabilitation Program
Designed to increase independence for those who no longer require acute care services but are often too complex medically to return home, the Short-term Rehabilitation Program helps residents recover and adapt to physical, neurological and psychosocial challenges, and provides a less costly and more comfortable alternative to continued hospitalization. Individual care plans facilitate speedy recovery and allows residents to return home as soon as possible.
Gouverneur provides care for the unique needs of terminally ill patients. Our interdisciplinary team of professionals and volunteers provide emotional support and pain control for patients and their families.
Long-term Residential Care
Gouverneur Skilled Nursing Facility provides a home-like environment with 24-hour care and support to people who require assistance because of physical, emotional or mental conditions.
Making the Right Choice
Choosing the right type of long-term or short-term care for a loved one - along with decisions about the cost of care - can often seem overwhelming. The staff at Gouverneur Skilled Nursing Facility recognizes that the transition of going from a home, assisted living, or a hospital setting into a Skilled Nursing Facility can be a difficult one, both financially and emotionally. The Social Work Departments and business office at Gouverneur make this transition as smooth as possible for the resident and family. We are here to answer your questions and walk you through the admission process.
Avoid Surprises. Reduce your chances of receiving an unexpected invoice for non-emergency healthcare. Get information about how to reach our financial counselors, what health plans we participate in and the costs for many of our services.
The Admission Process
