W-94 Install, Alteration, Test, & Repair of Flammable & Combustible Liquid Storage Systems

The W-94 Certificate of License is required for the installation, alteration testing & repair of flammable and combustible liquid storage systems.
This Certificate of License is valid only for the specific person to whom it is issued and can be used anywhere within New York City.

Certificate of Fitness Requirements

W-94 Certificate of License can only be obtained by meeting the special qualification requirements. 

All applicants should review the Notice of Exam.

Qualified applicants should review the AIP information.

The following documents are also needed

  • A completed Certificate of Fitness Application Form (PDF)
  • A letter must have a minimum of three (3) years’ work experience in the installation, alteration, testing or repair of automotive or marine liquid motor fuel storage and dispensing systems in the five (5) year period prior to the date of filing of such application. Such experience shall have been obtained working under the general supervision of a holder of a certificate of license holder for automotive or marine liquid motor fuel storage and dispensing systems and flammable liquid and combustible liquid storage systems.
  • Reside in New York City or maintain a place of business in NYC.
  • Liability insurance of not less than $500,000, naming the City of New York and the Fire Department as additional insured.
  • If required, a copy of your diploma or academic transcript.

  • Two forms of identification, at least one must be government-issued photo identification, such as a driver's license or a passport.
