A subset of common FDNY records may be requested using the forms listed below.
Please read each form’s instructions carefully before submitting your request.
Property Records
Patient Records
The FDNY only maintains Pre-Hospital Care Reports for patients treated or transported by FDNY EMS. These records were previously known as Ambulance Call Reports (ACR). For all other patient records, contact the responding ambulance provider or receiving hospital.
If an invoice was issued for emergency transportation, it will list the name of the ambulance provider – It might not be the FDNY, and in that case you will need to contact the listed provider.
Certain requests for Pre-Hospital Care Reports must also include a completed HIPAA release form.
Freedom of Information Law (FOIL) requests to the FDNY should be submitted via NYC OpenRecords.
In NYC OpenRecords, select “Request a Record,” and then select “New York City Fire Department (FDNY)” from the agency menu.
Please note that all FOIL requests for record types listed under the “Records Request Forms” section (above) will be closed, and customers will be directed to use those forms instead.
Fire Incident Records
Fire incident and investigation records may only be requested via FOIL. For the quickest resolution, these records requests should be logged in NYC OpenRecords.
Alternatively, the following forms may be used to request these records in-person or by postal mail:
FOIL Officers
Inquiries about existing FOIL requests should be sent directly through NYC OpenRecords (under the request, select “Contact the Agency”)
