Court Case or Arbitration Notification Form

Fill out this form to notify the NYC Department of Consumer and Worker Protection (DCWP) when you file a case in court or a private arbitration for violations of the Protected Time Off Law, Fair Workweek Law, Delivery Worker Laws, Freelance Isn't Free Act, or another law that DCWP enforces. If the law requires you to serve on DCWP a copy of the complaint you filed in court, you can complete this online form to satisfy that requirement.
