Bingo Game Operator License Application Checklist

Who Must Have a Bingo Game Operator License?

A Bingo Game Operator license is required if an entity conducts bingo games in which designated numbers or symbols on a card are matched with numbers or symbols selected at random and prizes are awarded. Generally speaking, only organizations raising funds for the promotion of bona fide charitable, educational, health, and religious causes, and organizations of veterans or volunteer firefighters can obtain a Bingo Game Operator license.

This description is only a general explanation of which organizations must have a Bingo Game
Operator license.

Note: New York City has lower prize limits than New York State allows. In New York City, the prize limit is a maximum of $250 for a single game and not more than $1,000 per occasion.

License Requirements

The Department of Consumer and Worker Protection (DCWP) will deny your application if you fail to provide the required documents and information.

BEFORE YOU FILE your DCWP license application, you must do the following:

Obtain Bingo Identification Number from the New York State Gaming Commission.

Obtain Required Organizational Documents.

You must have the applicable documentation based on your organization’s legal structure. Although DCWP does not require submission of these documents in order to process your application, DCWP may request this information under section 20-104 of the New York City Administrative Code.

Not-for-profit corporations must register and remain active with the New York State Department of State. You can check your status at www.dos.ny.gov/corps.

If applicable, you must also have an Assumed Name Certificate for your organization.

WHEN YOU FILE your DCWP license application, you must submit the following materials:

Proof of Right to Use Premises

Submit ONE of the following documents as proof of right to use premises where you will conduct bingo games:

  • A copy of your deed or bill of sale OR
  • A current lease. Note: The landowner must have a Commercial Lessor license issued by DCWP.

Members’ Affidavit of Good Moral Character

Every member of the license applicant organization must complete and sign.

Granting Authority to Act Affirmation

Required if someone other than the license applicant will prepare and submit this application. The applicant must sign the license application and all related forms. Download Granting Authority to Act Affirmation.

License and Additional Fees

License Fee (due when you file your license application)

  • $18.75 per occasion
    • Note: The maximum number of occasions you can hold each year is 216. You cannot conduct bingo games on more than 18 days in a three-month period.

Additional Fee (due after you file your license application)

  • 3% of net proceeds

Pay by:

  • Credit card (Visa, MasterCard, American Express, Discover Card only) (Online or in person)
    You will be charged a nonrefundable 2% Convenience Fee.
  • eCheck (Online)
  • Checks from the applicant’s exclusive bingo account or money order payable to DCWP (In person or by mail)

Below is information you need to know after submitting your license application. All New York State Gaming Commission forms are available at gaming.ny.gov.

Application to Amend Bingo License

After you receive your Bingo Game Operator license, you may conduct bingo game activity. However, if you need to make changes to the license application, file BC-6 Application to Amend Bingo License 10 days before the originally declared occasion date OR within 72 hours after an emergency cancellation (e.g., snowstorm, hall closure).

Financial Statements, Cash Control Report, and Additional License Fee

You must submit BC-7 Financial Statement of Bingo Operations, BC-7B – Cash Control Report For The Conduct of Bingo, and pay an Additional License Fee of 3% of net profits to DCWP within seven (7) days of each bingo occasion.

Quarterly Summary Statement of Bingo Operations

You must complete BC-7Q Quarterly Statement of Bingo Operations within 15 days after the end of each calendar quarter. Submit the original to the New York State Gaming Commission (address on form) and one copy to DCWP (address above).
