Request an Annual Disclosure Report

You must submit a separate Annual Disclosure Report Request Form for each filer for whom you are requesting a report and upload a copy of your current government-issued identification below. If available, the Conflicts of Interest Board will produce the public portion of the filer’s original submission and any amendment(s) the filer submitted for the selected year.

Pursuant to the Annual Disclosure Law, the Board must notify the filer of the production of their report and of the identity of the person to whom the report was produced. Accordingly, your name and affiliation (where applicable) will be sent to the filer when the Board produces the report(s) to you.
