Patient-Centered Medical Homes

A patient-centered medical home (PCMH) is a better, easier way to get the primary care our community needs. A medical home is not an actual place.  It is a partnership that allows patients, their doctor, and their care team to get to know each other and work together to improve the patient’s health.  At HHC, we practice a patient centered medical home approach to care in all our primary and pediatric health centers across the five boroughs. More than 600 HHC primary care physicians and their teams have received PCMH Level 3 designation, the highest ranking, meaning that we meet or exceed safety and quality standards.


A New Way for Better Care

HHC Patient Centered Medical Homes provide a dedicated care team who work with patients and create a unique care plan to meet their healthcare goals.  Patients don’t have to come to the health center every time they have a question about their health.  The care team follows the patient’s health progress, and keeps track of tests and health screenings they need to make sure patients receive the right care at the right time.  Working with patients, our PCMH teams create a personal care plan to help patients reach their healthcare goals, whether it’s losing weight, staying up-to-date on screenings and vaccinations or quitting smoking.

A patient-centered medical home promotes routine, preventive care. For example, we:

  • Help manage and coordinate any screening, testing or treatment a patient may need
  • Coordinate visits to specialists and follow up with patients regarding their care
  • Create a health plan in collaboration with the patient and support them in reaching their health goals
  • Connect patients with the resources they need, such as a support group or health class 
  • Are available if patients need to reach us, whether they have a question or just need extra support.
At HHC, patients are active members of their care teams.  We encourage patients to talk openly about their health needs, ask questions, and do the best they can to follow the care plan they have agreed to. Our PCMH teams create an environment of respect, collaboration and communication. With a patient-centered medical home, patients can build a long-term relationship with their doctor, and the care they receive can help keep them healthy and out of the hospital.
