Comprehensive Nursing Services
Gouverneur's 210-bed Skilled Nursing Facility provides 24-hour care for individuals in need of short-term or long-term care. Our residents receive medical, nursing and rehabilitative care in an atmosphere of dignity and compassion. Gouverneur's multilingual, nurturing staff is committed to integrity, responsibility and excellent care, while emphasizing concern for the individual's physical, psychological, spiritual and social quality of life. Our program reflects the ethnic diversity of Manhattan's Lower East Side and has a reputation of excellence for more than 30 years. We are Medicare and Medicaid certified and accredited by the Joint Commission on Accreditation of Health Organizations (JCAHO) and by the New York State Department of Health (NYSDOH).

Our interdisciplinary teams of healthcare professionals develop customized care plans for each resident. Our Many Levels of Care
Short-term Rehabilitation Program
Designed to increase independence for those who no longer require acute care services but are often too complex medically to return home, the Short-term Rehabilitation Program helps residents recover and adapt to physical, neurological and psychosocial challenges, and provides a less costly and more comfortable alternative to continued hospitalization. Individual care plans facilitate speedy recovery and allows residents to return home as soon as possible.
Hospice Care
Gouverneur provides care for the unique needs of terminally ill patients. Our interdisciplinary team of professionals and volunteers provide emotional support and pain control for patients and their families.
Long-term Residential Care
Gouverneur Skilled Nursing Facility provides a home-like environment with 24-hour care and support to people who require assistance because of physical, emotional or mental conditions.

The Admission Process
The admission process involves several steps and requires you to provide certain information, including a complete medical background, a physician's order for placement, financial and insurance information, and other pertinent information.
