Cash Assistance 6-month Mailer

Need to submit your CA 6-month mailer?

Follow these steps below. 

First, did you receive the 6-month mailer IN THE MAIL? 

If YES:

• Fill out the paper form completely, sign and date it. Then, return the paper form to us. The fastest way to submit the form is to upload it by mobile app. 
  • Looking for the ACCESS HRA mobile app? You can access it from this page on your mobile phone or tablet. It's free on iOS or Android devices:
    • To download the app:
    1. Go to the App Store or Google Play
    2. Search for “ACCESS HRA”
    3. Click on GET or INSTALL

If NO:

• Do you have a printer?
• If yes:
• Go to nyc.gov/accesshra, log-in, and complete account set-up to view your NOTICES. The 6-month mailer should be available to you as an-e-notice. Click to open the PDF and print it. If it does not appear there: Click here for a blank copy of the 6-month mailer form to print.

• If no:
  • Call Infoline at 718-557-1399 to request that a paper form be sent in the mail.
