W-79 Delivering, Installing, Repairing Cryogenic Containers or Systems

Certificate Description

A Certificate of Fitness is required to person who delivers and installs cryogenic containers and /or performs maintenance and minor repairs on cryogenic systems.  
 
This Certificate of Fitness is valid only for the specific person to whom it is issued and can be used anywhere within New York City.  

Cost of Certificate of Fitness 

• New Application Fees: $25
• Renewal Fees: $15
• Application Reviewed Within: 1-2 Days
• Renewal Cycle: 3 Years

The following documents are also needed 

• For applicants who need to apply for the W-79 exam:
  
  • Two forms of identification, at least one must be government-issued photo identification, such as a driver's license or a passport.
  • A completed Certificate of Fitness Application Form (PDF)
  • A letter of employment.  The W-79 applicant must be employed by a company which is in charge of delivering or installing or repairing cryogenic containers of systems.

• For applicants who need to apply W-79 through Alternative Issuance Procedure:
  • A completed Employee Applicant Affirmation Form (PDF). Applicants must have this form notarized.
  • A copy of the required License or diploma (see W-79 Alternative Issuance Procedure Information. PDF)
