F-61 Supervision of Hookah Establishments

About

The use of non-tobacco hookah must be supervised by a person holding a F-61 Certificate of Fitness for the following activities:

  • safe storage of charcoal
  • ensuring noncombustible surfaces in the charcoal preparation area and in the smoking area
  • safe handling of lighted charcoal and the disposal of spent charcoal and other combustible waste

This Certificate of Fitness is valid only at a specific location and for the specific person to whom it is issued.

  • New Application Fees: $25
  • Renewal Fees: $15
  • Application Reviewed Within: 1-2 Days
  • Renewal Cycle: 3 Years

Before You Submit Your Application

You must pass a written exam to get an F-61 Certificate of Fitness. All applicants should review the Notice of Exam and Study Materials.
