NYC REACH

NYC REACH assists New York City-based primary care providers and practices with optimizing health information systems, quality improvement and value-based initiatives.

NYC REACH partners with organizations and providers throughout New York City to:

• Support New York City’s health goals around prevention, primary care and COVID-19 recovery
• Facilitate connections between communities and clinical resources
• Optimize use of health information technology and adapt data to facilitate improvements in patient care and healthcare
• Translate federal, state and local policies and programs into action (such as the Quality Payment Program, Patient-Centered Medical Home and Medicaid Waiver)

NYC REACH is in the Bureau of Equitable Systems in the Center for Health Equity and Community Wellness Division. NYC REACH strives to support primary care practices in delivering high quality, person-centered, healthcare worker-affirming, anti-racist and coordinated care for marginalized populations.
