The Identifying Information Law (Local Laws 245 and 247 of 2017) sets forth requirements for city agencies to follow in the event that agency collection and/or disclosure of personal identifying information constitutes a breach of security. If you know or have reason to believe that your personal identifying information has been collected or disclosed in a manner inconsistent with the requirements of the Identifying Information Law and not in the ordinary course of the agency’s business, please file a complaint by e-mailing Qn01@cb.nyc.gov.
The agency privacy officer (APO) will receive and investigate any complaints promptly. The APO will notify the Chief Privacy Officer of the complaint with 24 hours of discovery. Following a complaint, the APO will work with and share information with internal legal, program, technical, or other staff, including the Chief Information Security Officer, and may engage other City offices, including the Chief Privacy Officer, the Law Department, Cyber Command, the Office of the Queens Borough President, and others that can help investigate the complaint and advise on a response.
