Medical Examiner Casefile Records Requests by Government Agencies/Organizations

For law enforcement, law offices, legal organizations, government agencies, and other organizations requesting records for investigative purposes:

You may submit a report/records request by following one of these two options:

1. Electronic Submission: You may complete the OCME Reports Request (Government Agency/Organization Requests Only) webform which will send your request to the OCME Records Request group.
   
   
2. Manual Submission: You may download the OCME Reports Request (Government Agency/Organization Requests Only) form and follow the instructions on it to complete and submit your request.

Note: Please use this form if you are requesting to view and/or photograph microscopic slides or are requesting recut slides.
