Medical Examiner Casefile Records Requests by the Next of Kin

For family members and next of kin, OCME can only provide the following upon request:

  1. Autopsy report only (all laboratory reports included by reference)

You may submit a report request by following one of these two options:

  1. Electronic Submission: You may complete the OCME Records Request (Family/Next of Kin Requests Only) webform which will send your request to the OCME Family Records Request group.

  2. Manual Submission: You may download the OCME Records Request (Family/Next-of-Kin Requests Only) Form and follow the instructions on it to complete and submit your request.

Note: Records may only be requested by next of kin or officers of the court. Each person requesting an autopsy report should submit only one request. Requests can take approximately 3 - 6 months to fulfill.

If you want these reports sent to someone other than yourself (e.g., to an attorney or an insurance company), your signature on the manual submission request must be notarized.
