This form is to be used only by law enforcement agencies, government agencies, and other organizations requesting records for investigative purposes.

The ME case number format for deaths reported in the Bronx is: B-YY-######. Use "B" for "Bronx County." Replace "YY" with the two-digit year and then enter the six-digit case sequence.

The ME case number format for deaths reported in Brooklyn is: K-YY-######. Use "K" for "Kings County." Replace "YY" with the two-digit year and then enter the six-digit case sequence.

The ME case number format for deaths reported in Manhattan is: M-YY-######. Use "M" for "Manhattan." Replace "YY" with the two-digit year and then enter the six-digit case sequence.

The ME case number format for deaths reported in Queens is: Q-YY-######. Use "Q" for "Queens County." Replace "YY" with the two-digit year and then enter the six-digit case sequence.

The ME case number format for deaths reported in Staten Island is: R-YY-######. Use "R" for "Richmond County." Replace "YY" with the two-digit year and then enter the six-digit case sequence.

Special Notes from the NYC Office of Chief Medical Examiner
  • If you request an in-person pick up, then the Records Department will notify you when the reports are ready.
  • You may contact the OCME Records Department Monday through Friday 8:00 AM to 5:00 PM by calling (212) 323-1777. The Records Department is closed on Federal Holidays but will reopen on the next business day.
  • If you leave a voice message, please clearly state your full name, the ME case number you are referencing and provide a phone number where you can be reached during the Records Department's business hours.
  • The final autopsy reports can take weeks to several months to finalize due to the detailed studies that may be performed. 
  • Note that the District Attorney must approve release of case material when the Manner of Death is deemed by the medical examiner to be a homicide.
