If you are requesting telephone hearing requests for more than three summonses, please upload a Word document below listing the summons numbers, dates of hearings and respondents' names.

Reminder: You should request a phone hearing before the hearing date listed on your summons. If you do not complete your hearing at the time and date listed on your summons or reschedule notice, you will be found in default and may face higher penalties and possible suspension of your license.

Nothing in this form will be submitted to the Hearing Officer assigned to your case and these upload fields are not to be used for your evidence. During your Phone Hearing, the Hearing Officer will give you the correct email address where you will be able to send your evidence.

NOTE: Once you press submit, you should immediately receive a confirmation that your submission was transmitted to the Clerk’s Office. You will then receive an email with your hearing date and time once the Clerk’s Office has had the opportunity to review your submission and has verified your information. The email will provide you with a case-specific PIN number and provide instructions on how to call in for your Phone Hearing on your hearing date. Please check your spam/junk folder if you have not received an email by the day before your hearing.
