Community Support Network

The Community Support Network connects New Yorkers to physical and mental health resources. The community health workers at partner organizations can make referrals to providers who can help with:

  • Daily concerns related to physical health
  • Concerns about safety at home and/or in the community
  • Strategies to deal with stress, anxiety and emotional health
  • Connections to food and housing
  • Connections to education, career and job-readiness
  • Connections to substance use supports and treatment

Everyone experiences daily struggles that cause stress. Daily stress builds over time and may become tough to handle alone.

Burial Assistance

The Human Resources Administration’s Office of Burial Services helps individuals in need of financial assistance to meet funeral expenses for a deceased low-income NYC resident.

OBS can pay up to a maximum of $1,700 towards the cost of a funeral bill.

Total burial cost cannot exceed a cap of $3,400. To determine eligibility:
