If you believe that NYC Aging or one of its contractors has collected, retained, or disclosed identifying information in violation of the Identifying Information Law (Admin. Code §§ 23-1201 through 23-1205), you may fill out and submit this form.

Specify the information you believe was disclosed in violation of the Identifying Information Law.
If the complaint is against a contractor, include the contractor name.
Indicate whether you have made a complaint with any other City agency, including NYPD, and include the date on which you made the complaint.

Complaints will be sent to NYC Aging's privacy officer and will only be used to investigate this complaint. To investigate and resolve the complaint, NYC Aging may share complaint information with other City agencies.
