Primary & Preventive Care

HHC hospitals and medical centers are not just for emergencies or surgeries. We want to be your medical home for all of your healthcare needs, from prenatal care and well-baby visits to adolescent health services, chronic disease management and geriatric care. We have primary care centers in our hospital buildings and in stand-alone health centers in neighborhoods across the five boroughs. Our goal is to keep you healthy and improve your quality of life. With routine care, you can continue doing the activities you love.

 

A New Way for Better Care

Our primary care doctors work with a team of expert healthcare providers, including nurses, social workers and others, to manage all your healthcare needs. We call this a patient-centered medical home. It’s not an actual place, but a partnership that allows you, your doctor and your care team to get to know each other and work together to improve your health. Your team always knows what is going on with your health and helps you get the right care when you need it. Your medical home can offer many ways to help you improve your health and well-being, and prevent, treat or manage chronic conditions such as diabetes, heart disease or asthma.

HHC Patient Centered Medical Homes provide a dedicated care team who work with you to create a unique care plan to meet your healthcare goals. You don’t have to come to the health center every time you have a question about your health. Your care team follows your health progress, and keeps track of tests and the health screenings you need to make you receive the right care at the right time. We work with patients to create a personal care plan to help you reach their healthcare goals, whether it’s losing weight, staying up-to-date on screenings and vaccinations or quitting smoking.

A patient-centered medical home promotes routine, preventive care. For example, we:

  • Help you manage and coordinate any screening, testing or treatment you may need
  • Coordinate visits to specialists and follow up with you regarding your care
  • Create a health plan with you and support you in reaching your health goals
  • Connect you with the resources you need, such as a support group or health class
  • Are available if you need to reach us, whether you have a question or just need extra support.

At HHC, patients are active members of their care teams. We encourage patients to talk openly about their health needs, ask questions, and do the best they can to follow the care plan they have agreed to. Our PCMH teams create an environment of respect, collaboration and communication. With a patient-centered medical home, you build a long-term relationship with your doctor, and the care you receive can help keep you healthy and out of the hospital.
