Report Related to the Identifying Information Law

If you believe that OLR or one of its contractors has collected, retained, or disclosed identifying information in violation of the Identifying Information Law (Admin. Code §§ 23-1201 through 23-1205), you may fill out and submit this form.

Complaints will be sent to OLR's privacy officer and will only be used to investigate this complaint. To investigate and resolve the complaints, OLR may share complaint information with other City agencies.
