CEQR Forms & Templates

This page contains the most current versions of standardized forms, templates and guidelines used in the CEQR Process.

Forms

• EAS Short Form  – This form can only be used for Unlisted actions.  Instructions.
• EAS Full Form  – This form is for all Type I and Unlisted actions. Instructions.
• Jamaica Bay Watershed Protection Plan Form - For use by projects in the Jamaica Bay Watershed. Instructions
• City Type II Prerequisite Screening Form – This form is for use in conjunction with actions classified as Type II pursuant to the CEQR Rules of Procedure. However, this form should not be submitted to the Department of City Planning.

Templates (for use by lead agencies)

• Lead Agency Letter – This model document provides guidelines for what a Lead Agency Letter must contain.
• Negative Declaration – This template contains language required in a Negative Declaration.
