Respond to Request for Information on Standing Online

You may submit this form if you have received a letter from a hearing officer from the OATH Hearings Division that is directing you to provide more information to show that you are authorized to request a new hearing after a failure to appear (vacate a default).

  • Answer every question in the space provided.
  • This form must be received by the OATH Hearings Division within 10 business days of the date of the letter from the OATH Hearing Officer or your request will be deemed abandoned.
  • If you have more than 5 summons, you must submit your response by mail to OATH Hearings Division Special Motion Part, 66 John St., New York, NY 10038, fax at 212-436-0768, or in person at any OATH Hearings Division location.
  • If there are more than 5 summonses/notices, you must submit your response by mail by sending it to the address listed at the top of the hearing officer's letter or in person at any OATH Hearing Office.
  • Registered representatives must upload and submit a signed, completed authorization form in the space provided at the end of the form.
