In order to proceed with your scheduled orientation, it is mandatory to email a copy of all documents listed below to:
- Valid driver’s license/Learner’s permit
- Birth certificate or Naturalization
- Social Security Card
- High school Diploma/GED
- College Transcript (IF APPLICABLE)
- Military Service DD214 Form (IF APPLICABLE)
- Vehicle registration and proof of insurance (IF APPLICABLE)
- Marriage license/divorce documents (IF APPLICABLE)
- Verification Letter for Peace Officer/Special Patrolmen status (IF APPLICABLE)
- Screening documents (Screening Sheet Documents)
- DO NOT LEAVE ANYTHING BLANK. If something does not apply to you, please write NONE. All lines/boxes should be filled in.
Failure to submit documents & forms in timely manner will delay your process.
