In order to proceed with your scheduled orientation, it is mandatory to email a copy of all documents listed below to:

  • Valid driver’s license/Learner’s permit
  • Birth certificate or Naturalization
  • Social Security Card
  • High school Diploma/GED
  • College Transcript (IF APPLICABLE)
  • Military Service DD214 Form (IF APPLICABLE)
  • Vehicle registration and proof of insurance (IF APPLICABLE)
  • Marriage license/divorce documents (IF APPLICABLE)
  • Verification Letter for Peace Officer/Special Patrolmen status (IF APPLICABLE)
  • Screening documents (Screening Sheet Documents)
  • DO NOT LEAVE ANYTHING BLANK. If something does not apply to you, please write NONE. All lines/boxes should be filled in.

Failure to submit documents & forms in timely manner will delay your process.
