TPO, NYC! Hypertension Community Champions
TPO, NYC! Hypertension Community Champions help spread the word about high blood pressure and related services. These volunteers are actively involved in their communities and many work for local organizations.
Community Champions meet quarterly to share information and resources.

Brownsville Community + Clinical Linkages Project
TPO, NYC’s Brownsville Community + Clinical Linkages Project is a three-year project to address disparities in hypertension care by improving public health infrastructure. The work is based at the Brownsville Multiservice Family Health Center at Bristol, housed within the Brownsville Neighborhood Health Action Center.
