NYC Human Resources Administration (HRA) provides aftercare services through the Homebase program to individuals and families newly relocating from the DHS and HRA shelter systems with rental assistance or entering a City rental assistance program from the community.

Services will respond to the specific needs of families and individuals using a holistic and comprehensive case management service delivery model in order to provide each household with the necessary support from the point of lease signing through post move-in; ensuring a smooth transition to their new communities.

Homebase will provide case management and wrap-around services incorporating referrals to resources and information including legal services and other supports in the community. Homebase will develop a service plan with the family or individual that incorporates short and long term goals and links them to referral services in community. Services may include but are not limited to:

  • Communicate clear tenant roles and responsibilities regarding lease, rent and communication with landlord
  • Communication regarding eligibility for rental assistance; including with brokers, landlords and providers
  • Acclimate tenants to their new neighborhood
  • Familiarize tenants with nearby transportation, hospitals, schools, daycare, and other services
  • Familiarize tenants with community-based resources in neighborhood
  • Connect tenants to the Resident Economic Empowerment and Sustainability for economic and educational advancement
  • Employment readiness and advancement
  • Financial literacy
  • Benefits screening
  • Legal Services
