Co-Naming Guidelines

Community Board 13 has adopted the following Street Co-Naming Guidelines effective April 27, 2023. If you are interested in applying for Street Co-Naming, please print and complete the forms (Co-Naming Guidelines Form, Petition Form, and a letter of support from a Local City Council Member) in it's entirety and submit to Community Board 13 in person or to the following e-mails:
