Send the completed Enrollment and Referral Agreement Form back to your
Community Referral (HHC Connectx) Office contact.
In order to promote continuity of care, HHC encourages referring providers to
complete an Enrollment and Referral Agreement Form with their affiliated
HHC hospital. A completed and signed Enrollment and
Referral Agreement Form will be a requirement for providers to join
MetroPlus for the first time or go through the Metroplus recredentialing
process. It will also be a requirement to join HHC Connectx, with its many
benefits, including use of the Community Referral (HHC Connectx) Office at your
facility to expedite your patient referrals into our hospitals. The provider’s
use of the Community Referral (HHC Connectx) Office to direct patient referrals
facilitates the appropriate exchange of clinical information.
