Fiscal Year 2023 Initiative Final Reports

In Fiscal Year 2023, organizations will complete the Initiative Final Report for the following programs:

  • Anti-Gun Violence Initiative - Art a Catalyst for Change
  • Cultural After-School Adventures Program (CASA)
  • Cultural Immigrant Initiative
  • Coalition of Theaters of Color (CTC)
  • SU-CASA


Organizations receiving Fiscal 2023 support through these Initiatives must report on their funded services in order to retain the funds paid, receive any payments due, and remain in good standing for the future.  

A Final Report is required for each Initiative-specific Grant Agreement; your organization will report on multiple designations for the same Initiative on a single form. Please refer to your organization’s Initiative Final Report notification e-mail for a list of all FY23 Initiative Final Reports required of your organization, and to obtain the pre-populated Designation List that organizations will complete and attach to the online report submission.

Please contact your program officer immediately if the total project expenses for an Initiative are less than the total amount of the grant and/or if your organization was unable to present a proposed project.  

Submission of all Initiative Final Report forms are due no later than Tuesday, August 1, 2023 at 5:00pm (ET). In addition to the online form, web-based documentation of your funded Initiative activities must be included as part of your Report. Please do not mail hard copies of any documents to the DCLA office; these submissions will not be accepted.

The FY23 Initiative Final Report form must be submitted online. It is compatible with all web browsers, but cannot be saved. Once you start to fill it out, the form must be completed in full to submit. We recommend that you view the form in its entirety and prepare your responses offline before filling out the form online. You will receive an email receipt with a PDF copy after submitting the form online – please save this for your records in case your program officer requests revisions to your original submission.

Reports must be reviewed and approved by DCLA before final payment of FY23 Initiative funds can be made.
