    Businesses that are Corporations or Partnerships must have an Employer Identification Number (EIN). Sole proprietors may use their Social Security Number.

      Copy of E.I.N. or Tax I.D. number as provided by the Internal Revenue Service (IRS).

        Authorized Representatives To Obtain Permits: Enter all persons you authorize to obtain permits for you, their affiliation to you and their telephone number.

          Copy of Incorporated Papers, License, Business Certificate or Filing Receipt filed with the State Of New York

            If this permit is being issued in conjunction with a Department of Consumer and Worker Protection (DCWP) license, the license must have been received from DCWP and you must provide the license number on the registration form.

              Contractors must present original insurance certificates for Commercial General Liability (CGL) insurance and Worker's Compensation insurance. Permit Bond. Where applicable, an applicant must submit an original Permit Bond to the Permit Office. If you are hiring a contractor, they must provide the insurance requirements before a permit will be issued. Insurance documents must have the same name and address as the Company applying for the Permit.

                Exact diagram of work location you are requesting permit for showing all pertinent information including north arrow, sidewalk/street widths and distances to curbs, building lines, corners etc. which will help in showing the exact location of the work. These must typically be filed and sealed by a professional architect or licensed engineer.

                  Registered Permittees may apply, pay for and print their permits anytime anywhere using the NYCStreets Permit Management online system. www.nycstreets.net. Please note that although permits can be submitted, paid for and printed out 24/7, the permit review process occurs only during normal weekday Permit Office hours.
