	Welcome to the HHC Connectx Community Provider Resources Site. In recognition of the importance of partnering with our community-based providers in managing our patients' healthcare, HHC Connectx and HHC's Chronic Care Collaborative aim to offer resources and tools for management of select chronic illnesses. These resources were carefully selected specifically for our community-based providers in conjunction with HHC clinicians. They are from a range of sources including NYC Health and Hospitals Corporation, MetroPlus Health Plan, New York City Department of Health, federal agencies, and other renowned healthcare organizations. We hope that the information and tools help providers work with and empower their patients to effectively manage their healthcare.
