After you've submitted a volunteer application, you will have up to three weeks to submit two reference forms. These forms may be written by either professional or personal contacts (e.g., employer, co-worker, professor, teacher, neighbor, friend, pastor-no family members please). Submit completed reference forms via email (Christina.Cornacchia@nychhc.org), fax 718-616-4782 or in person.
