Request a Medallion Ownership Verification Letter

A Medallion ownership certification letter is a statement issued by the Taxi and Limousine Commission as proof of ownership of a Medallion.

Fill out and submit the below form to request a Medallion Ownership Verification Letter.
Email a copy of the owner’s government issued photo ID to TLC at Medownerverify@tlc.nyc.gov along with the medallion number for which you are requesting proof of ownership.
If you are not the owner, agent, or corporate officer, you must also email a Power of Attorney Form signed by the medallion owner to receive the verification letter by email. Link to Power of Attorney form.
Once your information is verified and the request completed, you will receive an email with the Medallion Ownership Verification Letter.
