TLC Medallion Storage Option

The TLC currently allows Medallion Owners to place Medallions and Rate Cards into storage by emailing a completed Medallion Storage Receipt form to the TLC. Instructions as well as the Medallion Storage Receipt form are included below. To remove a Medallion and Rate Card from storage, follow the applicable instructions below. Please note the different instructions for Medallions that did not surrender their plates while in storage, and for Medallions that surrendered plates or purchased a new vehicle while in storage.”

Are you qualified?

  • All Medallion licensees can put a medallion and rate card into storage for up to 60 days.

  • Completed Medallion Storage Receipt form.

How to put your Medallion / Rate Card into storage

    1. Print this storage form, Medallion Storage Receipt Form from the TLC website.

Download and print the Medallion Storage Receipt Form from the TLC website. Completely fill out the printed form. Incomplete forms will not be processed.

    1. Email the Storage Receipt Form to TLC.

Scan and email the Medallion Storage form to operationsm@tlc.nyc.gov. Please do not come to the TLC’s Long Island City Facility to submit documents. Documents will only be accepted by email.

    1. Medallion license status & email notification.

The medallion’s license status will no longer be active, and the status will be changed to “suspended.”  You will not be allowed to operate the vehicle while the medallion is in storage.  An email will be sent notifying the sender that the storage request was processed.
Please visit the NYS DMV website for instructions to surrender license plates by appointment, drop off, or by mail.

How to remove a Medallion / Rate Card from storage

To remove a Medallion / Rate Card from storage, please follow the instructions below.

If you still have your TLC plates / HAVE NOT surrendered TLC plates while your Medallion was in storage, follow these instructions:

  1. Request an updated certificate of insurance / liability from your broker or insurance carrier, with a current date of issuance on the certificate; then
  2. Email the Medallion Storage Receipt form used to place your Medallion into storage, the updated certificate of insurance / liability, original FH-1 insurance card and original registration, current vehicle inspection report, a copy of government issued photo ID, and a Power of Attorney form (if not the Medallion owner) to operationsm@tlc.nyc.gov.

If you HAVE surrendered TLC plates or purchased a new vehicle while your Medallion was in storage, please follow these instructions:

  1. Request an updated certificate of insurance / liability from your broker or insurance carrier, with a current date of issuance on the certificate;
  2. Schedule a Yellow Cab / Medallion Transaction appointment using TLC’s online appointment system; then
  3. Bring the Medallion Storage Receipt form used to place your Medallion into storage, the updated certificate of insurance / liability, original FH-1 insurance card and original registration, original title, bill of sale (or E-MV50), a copy of government issued photo ID, and a Power of Attorney form (if not the Medallion owner) to the scheduled Yellow Cab / Medallion Transaction appointment.
