Diabetes is a condition where your body cannot control the level of sugar in your blood.


  • Diabetes is the 4th-leading cause of death in NYC
  • The number of adults diagnosed with diabetes has tripled in the last 20 years

Prediabetes means your blood glucose (sugar) level is higher than normal but not high enough to be considered diabetes. People with prediabetes are at risk for developing type 2 diabetes within 5 years.


  • 1 in 3 adults are prediabetic – and most don’t know it
  • 100,000 New York City employees are at risk for developing diabetes.


The good news is that type 2 diabetes can be delayed or prevented in people with prediabetes through effective lifestyle change programs like the National Diabetes Prevention Program (NDPP).


WorkWell NYC brings NDPP to the workplace, providing convenient access to life‐changing classes that empower employees to lead a healthier lifestyle.

 

Over the course of a year, a lifestyle coach leads participants through discussions about:

 

Eating healthy Managing stress
Losing weight Staying motivated
Incorporating physical activity into your daily lifestyle  Coping with challenges and problem-solving
Establishing and maintaining healthy routines    

 


The NDPP can delay or reduce the risk of developing type 2 diabetes by as much as 58% through lifestyle changes resulting in modest weight loss of 5%-7%.


NDPP classes are currently occurring in several agencies across the city, including DOHMH, DOE, FDNY, ACS, NYCHA, and DOT! Yours could be next!


Am I eligible to participate in the NDPP?

Participants MUST:

 

 

  be at least 18 years old
✔  have a body mass index (BMI) that exceeds 25
  be diagnosed with prediabetes

 

 

Not sure if you are prediabetic? Visit your health care provider to have an A1C test performed or take the CDC Prediabetes Screening Test here. If you score a 9 or above, you may be at high risk for developing type 2 diabetes.


What to expect from the NDPP?

NDPP is a yearlong program that takes place at your worksite. Classes occur once a week for the first six months, then bi-weekly/monthly for the remaining six months. Throughout the course of the program, participants are equipped with essential tools needed to lead a healthier lifestyle.  Learn More

In order to have a successful experience in NDPP participants are expected to:

  1. Attend sessions on time
  2. Make up missed sessions
  3. Contribute to evaluation surveys
  4. Be a supportive peer
  5. Check in periodically with the NDPP Coach

 

 


How do I launch NDPP at my worksite?

A Site Ambassador is someone who is able to assist with program efforts such as setting up informational sessions, securing a meeting space, and marketing the program. The Lifestyle Coach handles the rest!

If you are interested in launching the NDPP at your worksite, please email WorkWell NYC at workwell@olr.nyc.gov with the following information:

 

 

  1. Your name and agency
  2. # of employees at your site
  3. Contact information for your HR department or agency head

 

 

Once we receive your submission, a team member will be in touch. Please keep in mind, a minimum of 15 participants are required in order to launch the NDPP at your worksite.

Nurse case managers and social workers work with you and your doctor to make sure you are getting the best care possible in managing your Type I, Type II or gestational diabetes.
