A1C Registry

An A1C test measures your average blood glucose level over a three-month period. Your health care provider can use your test results to diagnose diabetes or see how well you are managing your diabetes.

If your A1C level is high, you may be at increased risk for kidney, eye or foot problems. You can lower your A1C by:

  • Taking your medications as prescribed.
  • Eating more whole foods, such as fruits and vegetables, and fewer processed foods.
  • Keeping a healthy weight.
  • Increasing your activity by moving more and sitting less.

The Health Department uses AIC data to understand how New Yorkers with diabetes are managing their glucose levels. The Health Department may also use the A1C Registry to call or mail you information about important health programs.

How to Submit a “Do Not Contact” Request

If you do not want to be contacted or have your health care provider receive your AIC information through the Health Department’s program, you can submit a “Do Not Contact” request by:

This request will not affect the care you are receiving or the information your health care provider receives from the laboratory. You will still be able to receive resources from your provider.
