COVID-19 Survey
You have been invited to complete the NYC COVID-19 survey.
This survey aims to provide an estimate of how many people in NYC have had COVID-19, including those with mild illness who might not have been tested. It includes questions about recent health conditions, health care, ability to isolate, and bias or discrimination.
We are sending the survey to a group of Healthy NYC participants who have been selected at random. We will be conducting this COVID-19 survey on a regular basis. You may be invited to take this survey more than once, as we continue to learn more about COVID-19 and its impact in NYC.
