AEU3321: Certificate of Correction and Statement in Support of Site Safety Training

The AEU3321 form must be used to certify the correction of summonses issued for failing to comply with training requirements pursuant to BC Section 3321.1 and/or Section 3321.2.1. The form combines the AEU2 (Certificate of Correction) and the AEU20 (Statement in Support). The AEU2 and AEU20 forms will not be accepted for summonses related to the site safety training requirements.

To be used to designate multiple equipment users. If filed without a CD4, this is considered an amendment and amendment fees apply.

CD5: Suspended Scaffold Application

Formerly known as the Notification of Staging and Outrigger Beam Use by Special Riggers form.

This form is to be used for the installation of a 2-point suspension scaffold on outrigger beams. It must be submitted by a NYC Licensed Master Rigger or a Licensed Special Rigger.

CD6: Crane/Derrick & Work Platform Plumbness & Torque Notification/Anchor Bolt Pull Out Test

To be used to notify the Cranes and Derricks division of the results of the mandatory work platform plumbness and torque test and anchor bolt pull test by a qualified professional.

NOTE: This form is only used on CNs filed prior to March 1, 2019.

CD6 Form

CD7: Notification of Erecting, Climbing, or Dismantling of a Climber, Tower Crane, or Derrick

To be used to notify the Department's Cranes & Derricks Unit before raising or lowering a climber, tower crane, or derrick Changes include: Derricks have been added as an additional equipment type requiring the submission of a CD7 form when applicable.

NOTE: The CD7 form and current notification procedures will remain in use regardless of whether the job is filed before or after March 1, 2019.

CD8: Technical Report: Statement of Responsibility (OLD Form)

To be submitted with a Crane Notice (CN) application subject to an inspection by a NYS PE or RA under the provisions of Directive 14 of 1975 as applied to cranes and derricks.

NOTE: This form is only used on CNs filed prior to March 1, 2019.

CD8: Technical Report: On-site Crane Inspections (NEW Form)

File this form after any Crane Notice (CN) inspection is completed. It is filed for every new phase or assembly, including tower crane jumps or climbs (up/down). It is also filed when one year has passed since the last CN inspection. Only the updated CD8 with the specified revision date of March 2019 in the bottom right corner will be accepted. CD8-TR: Identification of Special Inspections

This form is filed in addition to a CD8 form when special inspections are required to be performed on structural elements that support the crane or derrick. It replaces the use of a CD8-TR form dated March 2019, which will no longer be accepted by Cranes and Derricks for new applications filed after April 22, 2024. The crane design applicant who filed the crane or derrick notice application for the job determines when and which special inspections are required in accordance with 1 RCNY 3319-01, NYC BC Chapter 17 and 1 RCNY 101-06. Special inspections shall only be performed by registered special inspection agencies, approved for the project by the crane design applicant. The crane design applicant may perform these special inspections only if the crane design applicant is a registered special inspection agency.

CD8-AD: On-site Crane Pre-Operation Inspections

File this form for any assembly, phase or jump in which a crane or derrick requires an assembly/disassembly director. It documents unassembled and assembled inspection components such as serial numbers, material testing reports, tower crane mast section plumbness and bolt torque. The pre-operational load test is reported on the CD8-AD form, as well as specific inspections to be performed by the assembly/disassembly director. CD8-AD forms are not required for the setup or breakdown of a self-erecting tower crane, provided the setup does not require a boom, mast or other attachment to be installed at the site.

CD12: Designation of Primary Lift Director

To be used to request an exemption from a Certificate of On-Site Inspection for equipment that satisfies the conditions listed in Exception #3 to 1 RCNY 3319-01(c)(1), such as a service crane, clamshell rig, pile driver or a crane meeting the requirements of a 48 hour exemption.

MC2: Mast Climber Device Application (NEW)

Mast climber owners must only submit the MC2 form in conjunction with also submitting the MC-3 form for an approved CN application, or it will not be accepted. All other past uses of this form which were specific only to the device must now be done in DOB NOW: Build. The MC2 should be submitted with supporting documentation, which must include a list of the mast's critical components, the owner's certification and a disclosure of the mast climber's history.

MC3: Installer and Remover 

The mast climber owner must submit the MC3 form to designate the party responsible for installing or removing a mast climber at a job site.

MC4: Mast Climber Notice Application

The NYS-licensed engineer who prepared the mast-climber plans must submit the MC4 with the plans to notify the Department that a mast climber will be used. Changes include: Section 7 – Additional information.

MC5: Certificate of On-Site Inspection

The NYS-licensed engineer who prepared the mast-climber plans must submit the MC5 to sign off on a mast climber to be used in New York City. This application must be submitted with supporting documents and photos that demonstrate that the mast climber has passed all prior inspections before use.

Easements & Restrictive Declarations

POC3: Professional and Owner Certification: Easement Agreement or Restrictive Declaration

The POC3 form is to be used when an easement or restrictive declaration is filed in connection with a professionally certified application. The professional must sign the POC3 form, which will contain either the professional's certification: 1) that the easement or declaration materially conforms to the Department's approved form, or 2) that the easement or declaration has been reviewed and accepted by the General Counsel's Office.

Notification

Crane Notification

Use the online Crane Notification Form to notify the Department of certain crane or derrick activities in accordance with Local Law 77 of 2017 and 1RCNY Section 3319-01(c)(5).

Injury/Fatality Incident Information

Use the online Injury/Fatality Incident Information Form to notify the Department of Buildings about any site incident that resulted in a fatality or injury to a construction worker or member of the public within three (3) business days.

In order to comply with Local Law 78 of 2017, the property owner, contractor, subcontractor, or person otherwise in control of a construction site must use the Injury/Fatality Incident Information form to report details of the incident within three business days to the Department.

Raising or Moving a Building Notification

Please use the Raising or Moving a Building Notification Form to notify the Department of Buildings of work related to raising or moving a building to comply with the recently updated Flood Resistant Construction requirements of the NYC Construction Codes, Appendix G.

In order to comply with AC 28-116.2.3.2 of the Administrative Code, the building permit holder must notify the Department of Buildings using this form at least 48 hours prior to starting any work related to raising or moving a building, which is subject to Special Inspection.

Permit holders can also notify the Department of Buildings of work related to raising or moving of a building by contacting the Department of Buildings Call Center at (212) 393-2550.

SCBP: Notification of Unsafe Conditions

The SCBP: Notification of Unsafe Conditions form is used to alert the Department of any unsafe conditions related to buildings or structures. Subsequent to this notification, per Article 217 of the 2014 Building Code, a TR15 and accompanying inspection report is required to be submitted to the Department. For any hazardous conditions please first call 311 or 911.

Owner Notification of Start of Work

Use the Owner Notification of Start of Work Form to notify the Department of Buildings at least 72 hours before starting an alteration of a building in which any dwelling unit will be occupied during construction.

To comply with Building Code 3303.10.2, the owner of a building must notify the Department at least 72 hours before starting any work requiring a Tenant Protection Plan. In order to comply with §28-103.27.2 of the New York City Administrative Code, the owner of an occupied multiple dwelling with a permit for the alteration of 10 per cent or more of the existing floor surface area of the occupied multiple dwelling or an addition to the occupied multiple dwelling must notify the Department at least 72 hours before starting any work pursuant to the permit.

In addition, owners must post and distribute a notice regarding Tenant Protection Plan for occupants. The notice must be distributed to each occupied dwelling unit and posted in the lobby and

  • on each floor within ten feet of the elevator; or
  • if the building does not have an elevator, within ten feet of or in the main stairwell on each floor.

Local Law 159 requires Owners to post or distribute a Safe Construction Bill of Rights. HPD has created a form for Owners to post or distribute that is available on their website.

Certify Correction

You must correct the violating condition and certify correction with the Department. To certify correction, submit:

  • an original Certificate of Correction (AEU2 Form);
  • proof of correction, such as photographs and receipts;
  • a notarized statement attesting to how the violation was corrected.

You can submit a Certificate of Correction once the violating condition is corrected. The hearing and certification processes are separate. To submit a Certificate of Correction request, use an eFiling account to log into DOB NOW at www.nyc.gov/dobnow and select the BIS Options portal. Certify Correction

You must correct the violating condition and certify correction with the Department. To certify correction, submit:

  • an original Certificate of Correction (AEU2 Form);
  • proof of correction, such as photographs and receipts;
  • a notarized statement attesting to how the violation was corrected.

You can submit a Certificate of Correction once the violating condition is corrected. The hearing and certification processes are separate. To submit a Certificate of Correction request, use an eFiling account to log into DOB NOW at www.nyc.gov/dobnow and select the BIS Options portal. TR8 Technical Report for Energy Code Progress Inspections

The progress inspector must inspect and compile supporting documentation for all inspections noted on the original TR8: Technical Report Statement of Responsibility for Energy Code Progress Inspections and on any subsequent TR8 statements filed as a result of post-approval amendments. The reports shall include the inspections and/or testing listed in Table I and/or Table II PROGRESS INSPECTIONS FOR ENERGY CODE COMPLIANCE and address all conditions applicable to the scope of work. Complete reports shall be submitted to the department upon request. Per 1 RCNY § 101-07 Approved Agencies (b) (3) Documentation, an approved agency shall maintain records of inspections and tests for at least 6 years.

This template is provided as a guide for the level of documentation expected in a TR8 Inspection Report. The template may be filled in or a similar format may be followed, if the required information is included.
