Patient Privacy Notice

The protection of your health information is very important to us at the New York City Health and Hospitals Corporation. NYC HHC's Privacy Notice is a document which describes how medical information about our patients may be used and disclosed, and how patients can exercise a number of new rights granted under the Health Insurance and Accountability Act (HIPAA).

We at the New York City Health and Hospitals Corporation encourage you to review this Privacy Notice and become familiar with how your personal health information will be used and safeguarded, as well as rights regarding the protection of your personal data. The information in the Privacy Notice is effective September 23, 2013.

If you have a privacy complaint, report or other privacy-related matter that you would like to bring to Mr. Gurin's attention, please do not hesitate to contact the OCC through HHC's toll-free confidential and anonymous Helpline at 1-866-HELP-HHC (1-866-435-7442), or by e-mail at CPO@nychhc.org. You may also report privacy-related matters by contacting the Facility Privacy Officer assigned to the HHC facility where you have sought treatment.
