If you would like information on available long term supportive services in New York City, contact your local borough office, they can be reached over the phone, via email or by making an appointment to visit one of five locations citywide. The five organizations listed below are contracted by the NYC Department for the Aging to operate a NY Connects program in the respective borough. Persons who are deaf or hard of hearing may call the NY Relay System 7-1-1.
